Reminders for our Valued Patients 

  • We only see patients 18 years or older.
  • Insurance cards & photo IDs are required at every visit.
  • Demographic and office forms must be signed yearly.
  • Most prescriptions are filled for one (1) year. If you have not been seen in the office in over a year, you will be asked to schedule an appointment before another refill is approved.
  • Medication refill responses may take up to three (3) business days. Please be aware that some medications require pre-authorization before insurance will cover them.
  • It is important to know if your insurance requires a referral. It is the patient's responsibility to make sure we have the correct referral, prior to the appointment.
  • We do our best to accommodate our patients. Please keep in mind that as a specialty office, our schedules fill up quickly.
  • If your appointment is at the Reston office, you must park in the parking garage to the right of the building, and take your ticket inside for validation. Please do not park in the parking lot across from the building.
